Job Overview:

Responsible for planning, organizing and carrying out nursing service according to the ADH Center policies and procedures, assuming responsibility for participant and staff activities on an assigned team.  Clinical supervision and oversight provided by the Clinical Director.

Primary functions in this role:

  • Provision and supervision of all nursing services
  • Coordinate the completion of, and ensuring the accuracy of, each participant’s assessment
  • Coordinate the development, ongoing review, and revision of each participant’s plan of care
  • Ensure that nursing notes are written at least one time per month or more often as necessary to reflect each participant’s health care status
  • Respond appropriately to any acute changes in a participant’s health or functioning
  • Provide appropriate health-care education for each participant and legally authorized representative
  • Assist as necessary in the delivery of other Services
  • Ensure ongoing education, training, and professional development of all Program staff that is relevant to care of participants
  • Obtain, review, and implement primary care provider orders
  • Coordinate the participant’s care with the primary care provider and, if applicable, community service providers
  • Supervise medication management services
  • Other duties as assigned by supervisory staff

Equipment:

  • Medical and participant care equipment as needed to meet the needs of Adult Day Health Center’s participants.

Working Conditions:

  • Indoors
  • Extensive participant contact.
  • Possible exposure to infection, blood and blood borne pathogens.
  • May work with hazardous/infectious waste.

Qualifications & Skills:

  • Must pass required employee physical before employment begins
  • Current RN license in MA, which must remain current throughout employment
  • Maintain CPR and First Aid certification
  • Minimum 2 years’ experience in a community nursing position, preferably working with the elderly
  • Computer proficiency required
  • Must pass criminal background check
  • Access to reliable transportation required
